Coordinator to lead our ASCA Cares initiative—an empowering support
program for independent living residents in our premiere retirement

Position Summary: The ASCA Cares Program Coordinator is responsible
for the daily oversight, growth, and success of the ASCA Cares
program, which provides support to independent living residents
through assessment, care plans, and personalized services. This
role blends care coordination, marketing, program development, and
quality assurance while ensuring compliance with billing and
documentation standards. The Program Coordinator also identifies
service gaps, promotes the program, and guides its evolution to
meet resident and organizational goals. Lead the assessment,
planning, and coordination of personalized care services for
residents, ensuring safe, respectful delivery and fostering strong
relationships through responsive support and accurate
documentation. Drive innovation in service offerings, optimize
workflows, and identify strategic opportunities to expand and
enhance the ASCA Cares program in alignment with resident needs and
organizational goals. Serve as a central connector among internal
teams, external providers, and community partners to ensure
seamless, integrated care and effective communication across all
stakeholders. Champion the ASCA Cares program through strategic
promotion, engaging materials, and community events that educate
and inspire participation among residents, families, and staff.
Maintain meticulous records of services and billing to uphold
compliance, support accurate reimbursement, and ensure organized,
legally sound client documentation. Uphold the highest standards of
care by monitoring incidents, participating in quality improvement
efforts, and ensuring full compliance with policies, procedures,
and regulatory requirements. Education and/or Experience:
Bachelor’s degree required in Social Work, Nursing, Public Health,
Gerontology, Human Services, or a related field; Master’s degree
preferred. Minimum 3-5 years of care coordination, case management,
or program development within senior living, healthcare, or
community-based services. Experience with program development or
start-up initiatives a plus. Knowledge of senior services, care
planning, and community resources. Proficiency in billing
practices, documentation standards, and regulatory compliance in
senior care or healthcare settings. Demonstrated success in leading
interdisciplinary teams, developing care plans, and coordinating
service across internal department and external partners. Ability
to work independently, prioritize effectively, and manage multiple
responsibilities. Strong communication, organizational, and
problem-solving skills. Proficiency in Microsoft Teams (Office,
