an independent provider of thorough health assessments for
individuals nationwide. Our mission is to deliver outstanding,
personalized care that enhances health, reduces costs, and empowers
individuals to lead fulfilling lives. With a strong foundation in
clinical assessment and care management, our extensive network of
clinicians transcends traditional barriers by meeting members in
their own environments. We focus on improving the quality of care
for older adults and other at-risk populations, ensuring better
health outcomes and identifying chronic conditions that may

utilize your skills and compassion to significantly influence the
health and well-being of others. Join our team and enjoy
competitive base pay along with incentive compensation and benefits
while making a positive impact in the community! Responsibilities
About the Role During visits that may last up to one hour, our

office visits. The Matrix Comprehensive Health Assessment (CHA)
aims to enhance care quality and allows us to address multiple care
gaps in a single appointment. Our providers are responsible for:
Conducting assessments for adults and geriatrics, which include
reviewing medical history, diagnoses, treatments, health education,
physician referrals, case management referrals, follow-ups, and
thorough documentation in line with our guidelines and protocols.

tests. Collaborating with physicians, case managers, social
workers, family members, key caregivers, and other medical
personnel as needed. Working alongside Primary Care Physicians
(PCPs) to provide patient education and follow up as required.
