Assesses the social, psychological, cultural, environmental, and financial situations, as well as, disposition needs, for each referred patient. Collaborates with patients, families, healthcare team members, and community agencies to develop and implement plans to address identified needs. Evaluates effectiveness of plans and initiates change as needed. Uses crisis intervention, problem solving model, community organization, and advocacy skills in identifying needs and resources in the hospital and community.Certification in Case Management (CCM, ACM RN-C) required within one year of eligibility. BLS required within 90 days of hire.Master's Required.

• In-depth knowledge and experience in palliative care principles, including goals of care and advance care planning.
• Strong verbal and written communication skills are crucial for conducting educational sessions, collaborating with diverse teams, and advocating palliative care.
• The ability to empathize with patients, families, and healthcare providers is vital for fostering a supportive and collaborative environment.
• Experience in designing and delivering educational programs, workshops, and training sessions.
• Understanding of adult learning principles to tailor educational content effectively.
• Familiarity with medical coding practices, particularly in the context of palliative care services.
• Ability to implement and manage changes smoothly, utilizing change management techniques to overcome resistance.
• Strong problem-solving skills to identify barriers to utilization and develop effective solutions.
