Job Description:
ChristianaCare is currently seeking a full-time Registered Nurse
for our Bariatric Surgery Program Manager located on our Wilmington
campus.Primary Function:To optimize coordination of services across
the continuum of care for patients enrolled in weight management
/bariatric surgery programs. To teach patients, families and
professionals the principal components of the treatment plan to
achieve weight loss. - To effectively transition patients from the
outpatient setting to the inpatient unit, with discharge to the
home setting and long-term follow-up. - To act as a liaison between
the surgeons, inpatient staff, discharge planning staff, insurance
carriers and the patient/family to promote the highest quality,
most cost-effective care. -Work ScheduleMon-Fri, day shiftNo
weekends or holidaysPrincipal Duties and Responsibilities:
- Acts as a clinical liaison between Christiana Care, patients,
clinicians, and other healthcare providers.
- Coordinates and documents patients' care plans, prioritizing
based on patient need.
- Reviews patient's surgery scheduled including all
readmissions.
- Conducts and coordinates bariatric education programs on a
variety of topics, in a variety of community and clinical
settings.
- Reviews, evaluates, and utilizes current research related to
bariatric surgery and weight management.
- Participates in multi-disciplinary team meetings and patient
care conferences.
- Maintains up-to-date information on innovative and successful
strategies to reach the target population.
- Assesses patient's/caregiver's learning needs and adapts
teaching methods to provide a thorough understanding that will
maximize successful clinical outcomes.
- Assesses support systems and sets realistic patient
expectations for service delivery &/or initiation of the
treatment plan.
- Regularly reviews areas of charting and maintaining patient
records and reports in accordance with policies and procedures and
in compliance with JCAHO standards.
- Ensures continuous compliance with MBSAQIP standards,
maintaining relevant pathways, protocols, staff, and patient
education.
- Monitors and collects statistical data /outcomes as required in
collaboration with MBS Clinical Reviewer.
- Facilitates medical information, progress toward therapeutic
goals, errors, patient reactions, or complaints to appropriate
personnel in a timely manner.
- Takes personal responsibility for accurate and timely
completion of all necessary patient care documentation.
- Functions as an active member of the multi-disciplinary team;
communicates/contributes, receives information, acts on decisions
reached via the Team Conference.
- Collaborates with management team of the Surgical Unit and the
Unit Based Medical Director regarding administrative issues that
require intervention and/or resolution
- Shows skill in customer service principles and leads by example
in exhibiting these skills in dealing with all customers.
- Is proactive in addressing cost containment measures and is
aware of the need for operating in a cost-effective manner.
- Maintains established departmental policies, procedures, and
objectives regarding safety, environmental and infection control
issues. - -
- Initiates and actively participates in performance improvement
(PI) activities and quality assurance.
- Enhances professional growth and development through
participation in inservice or educational programs.
- Participates in marketing activities as needed.
- Demonstrates skills and knowledge necessary to provide
appropriate care to adult, and geriatric patients, including the
ability to obtain and interpret clinical information and to
identify patient needs.
- Performs assigned work safely, adhering to established
departmental safety rules and practices. - Reports to supervisor,
in a timely manner, any unsafe activities, conditions, hazards or
safety violations that may cause injury to oneself, other
employees, patients or visitors.
- Performs other duties and responsibilities as
directed.Education and Experience Requirements:
- Licensed / Registered clinician with minimum 10 years'
experience in healthcare delivery.
- Prior experience in weight management programs required, with
emphasis on Bariatric Surgery preferred.
- Experience in case management preferred.
- Certified Bariatric Nurse (CBN) preferredSpecial Requirements:
- Current RN licensure (or eligible to reciprocate) in the state
of Delaware
