Care Advantage Inc, Voted #1 in Home Healthcare the last 5 years is looking for a Licensed Practical Nurse (LPN) to join our Personal Care nursing team in a Part-Time capacity.Our team provides 1 on 1 care in patients' homes with extensive services to provide our patients with the best possible care. This position promotes and supports the Company's core values of Compassion, a positive Attitude, Respect and Excellence.We offer a competitive Hourly rate and a great work environment that is second to none.Our Licensed Practical Nurses assist with supervisory visits and follow-up visits based off the plan of care for patients located within the Newport News, Chesapeake and Portsmouth area as well as some of the surrounding areas.The Licensed Practical Nurse will effectively interact with patients, caregivers, family members and other interdisciplinary co-workers to provide nursing care to patients on an intermittent basis.A DAY IN THE LIFE OF A Licensed Practical NurseMakes initial contact with the patients and explains services offeredCompletes follow-up visits as neededInstructs assigned care team members on the needs and care plan detailsReports progress of care to involved partiesFollows up and maintains relationships with clients, always advocating on their behalfEstablishes relationships with community partners and referral sourcesKeeps Clinical Leadership and Office Administrator abreast of patient condition and/or medical changesReviews caregiver documentationTakes part in interdisciplinary team meetings to ensure needs of all patients are metCare Advantage Inc. is fully committed to enhancing and restoring the lives of our patients in the communities we serve. We are the largest privately owned home care company in the Mid-Atlantic with a wide range of services, programs and resources geared towards restoring the health and functional ability of our patients.Required Qualifications:Graduate of an approved school of professional nursingCurrent and valid Virginia LPN LicenseTwo (2) years' Personal Care home health nursing experience, preferred.Previous experience completing Supervisory visits, strongly preferred.Passionate and motivated to provide care within a patient's home.Ability to exercise initiative and independent judgment.Comfortable working collaboratively in cross-functional teamsAbility to communicate effectively, both verbally and in writing, with all levels of an organizationAbility and willingness to travel within a given territory.Willingness to travel to multiple patients in varying home locations.If this sounds like the perfect opportunity for you, please apply today!
